What you will do

• Serve as the clinical wound care resource for home health field clinicians and provide consultation on complex cases.
• Review wound assessments and clinical documentation for treatment planning, agency compliance, regulatory standards, and evidence-based practice.
• Collaborate with physicians, nurses, case managers, therapists, dietitians, and other interdisciplinary partners on individualized wound care plans.
• Provide recommendations for acute and chronic wounds, including pressure injuries, diabetic foot ulcers, venous and arterial ulcers, burns, surgical wounds, and skin tears.
• Develop and promote standardized wound care protocols, educate clinicians on treatment techniques, dressing selection, infection prevention, and ostomy management.
• Monitor healing outcomes, conduct clinical reviews, support utilization management, and identify quality improvement opportunities.

Requirements

• Active Registered Nurse license in Florida required.
• Previous home health nursing and wound care experience required.
• Knowledge of home health regulations, Medicare guidelines, OASIS, and clinical documentation requirements.
• Knowledge of evidence-based wound care, skin integrity management, and ostomy care.
• Strong clinical assessment, critical thinking, communication, and collaboration skills.
• Proficiency with electronic medical records and clinical documentation systems.

Nice to have

• Wound Care Certification such as WCC, WOCN, CWCN, or equivalent.
