What you will do

• Manage a panel of members with out-of-control diabetes, guiding them toward clinical stability and better self-management.
• Conduct detailed assessments and provide ongoing education on medications, glucose monitoring, and lifestyle counseling.
• Collaborate with nurse practitioners, social workers, and pharmacists to optimize care plans and medication regimens.
• Utilize motivational interviewing to influence positive and sustained behavioral changes in members.
• Create and track collaborative SMART goals for members to achieve clinical stability.
• Screen members for clinic eligibility and maintain detailed interaction documentation.

Requirements

• Must be located within one of the contiguous 50 US states.
• Fluent in both Spanish and English.
• Active unrestricted Compact Registered Nurse (RN) license or Registered Dietitian license.
• Active Certified Diabetes Care and Education Specialist (CDCES) certification.
• Minimum 2 years of experience in clinical diabetes management.

Nice to have

• Experience managing diabetes in a remote setting.
• Experience with diabetes technology, such as CGMs and insulin pumps.
