Focus on monitoring high-risk pediatric patients, addressing complex mental health and social needs, and connecting families with appropriate community and healthcare resources.

Location: Remote within the United States; Monday to Friday schedules of 8:00 a.m.–5:00 p.m. or 10:30 a.m.–7:00 p.m.; 10% travel required for training and education.

Salary: $70,000–$77,000 base salary, plus annual bonus incentive and benefits.

What you will do

  • Monitor high-risk pediatric patients up to age 21, including patients recently discharged from hospitals, and adjust care plans as needed.
  • Conduct biopsychosocial assessments and develop, evaluate, and update patient and family care plans.
  • Provide behavioral health interventions, psychoeducation, crisis de-escalation, and stabilization support.
  • Help families navigate mental health diagnoses, co-occurring medical conditions, social services, and behavioral health resources.
  • Coordinate with pediatricians, nurses, care coordinators, schools, specialists, child welfare agencies, and other external care teams.
  • Participate in interdisciplinary consultations and monitor patient progress.

Requirements

  • Master’s degree with major coursework in social work or a related field required.
  • LMSW required.
  • 3–5 years of post-graduate experience in healthcare social work or behavioral health case management required.
  • Experience working with pediatric populations and family systems.
  • Knowledge of behavioral health, pediatric and adolescent mental health, crisis intervention, motivational interviewing, goal setting, and trauma-informed care.
  • Experience providing telehealth services, proficiency with online platforms and Microsoft Office, and availability for afternoon or evening schedules.
