Salary: $99,000–$121,000 annual on-target earnings, including 20% tied to uncapped monthly commission.

What you will do
- Conduct consultations to understand client needs, urgency, readiness, and appropriate care plans.
- Guide families from inquiry to service start, align expectations, and coordinate onboarding.
- Manage reassessments, care plan adjustments, ongoing engagement, and retention.
- Partner with scheduling, operations, and Care Professionals to resolve execution challenges and maintain service quality.
- Maintain accurate documentation, track portfolio performance, and use CRM and operational systems.
- Support referral relationship growth and the reputation of the local market.
Requirements
- At least 1 year of experience in a consultative, client-facing role such as healthcare, senior care, social work, or care coordination.
- Experience guiding clients through decisions and owning measurable outcomes such as conversion, retention, or service adoption.
- Strong relationship management skills and the ability to adapt support as client needs evolve.
- Experience leading structured discovery, needs assessments, recommendations, and next-step conversations.
- Ability to collaborate cross-functionally, manage follow-up, maintain accurate records, and work with CRM and documentation systems.
- Reliable transportation and the ability to spend approximately 75% of working time in the San Francisco field, with occasional evening and weekend flexibility.
Nice to have
- Experience working with older adults and families.
- Background in healthcare administration, social work, gerontology, or a related field.
- Experience with Salesforce or a similar CRM.
