Licensed Social Worker (Healthcare)

Licensed Social Worker (Healthcare): Managing oversight of patients’ care within the Structured Family Caregiving (SFC) program with an accent on patient advocacy, compliance, and home-based assessments. Location: Hybrid (Field-based in Cleveland, OH). Must be based in Ohio and able to commute to the Cleveland office at least twice a month.

Salary: $28.00 – $33.00 per hour

What you will do

• Conduct initial in-person home assessments for patients and caregivers to ensure compliance.
• Provide monthly monitoring via telephonic and in-person visits to ensure quality of care and caregiver support.
• Maintain detailed and accurate case records and documentation according to agency policy.
• Serve as an escalation point and advocate for patients and caregivers within the SFC program.
• Ensure adherence to HIPAA, Ohio Department of Medicaid, and Ohio Department of Aging regulations.

Requirements

• Valid and active Ohio Licensed Social Worker (LSW) license issued by the CSWMFT Board.
• Bachelor’s (BSW) or Master’s (MSW) degree in Social Work from a CSWE-accredited program.
• Minimum 2 years of clinical experience, including 1 year working with elders or adults with disabilities.
• Valid driver’s license and an insured vehicle for travel within the state of Ohio.
• Proven experience in case management within a fast-paced, autonomous environment.

Nice to have

• Prior experience in home care.
• Bilingual proficiency in Spanish.
• Experience using CRM systems, preferably Salesforce.
