What you will do

• Review clinical documentation (SOC, RA, Care Plans) for completeness, accuracy, and compliance with NY State regulations.
• Collaborate with field nurses to correct missing or inconsistent records.
• Manage documentation workflows and track status using Medflyt.
• Prepare and submit clinical records for physician review and signature.
• Conduct quarterly clinical record audits and identify recurring documentation trends.
• Provide feedback and education to field nurses on documentation best practices.

Requirements

• Active and unrestricted New York State Registered Nurse (RN) license.
• Minimum one (1) year of home care experience.
• Deep knowledge of New York State Department of Health requirements and regulations.
• Ability to work in a hybrid setup in New York City.
• Proficiency with electronic medical records or workflow tracking systems.

Nice to have

• Experience in a Licensed Home Care Services Agency (LHCSA).
• Prior experience in chart auditing or quality assurance.
