Salary: $34.00 - $40.86 per hour

What you will do

• Consult with members to establish goals, develop personalized care plans, and monitor progress towards health outcomes.
• Proactively monitor clinical risk and deploy appropriate risk mitigation interventions.
• Empower members with self-management education to prevent disease progression outside of acute care settings.
• Collaborate with a multidisciplinary care team to deliver coordinated, whole-person clinical support.
• Identify behavioral health and social determinants of health needs and connect members to appropriate resources.
• Maintain HIPAA compliance and document all member interactions and nursing care plans accurately.

Requirements

• Active, unrestricted RN license (multi-state licensure required).
• Must live in a compact state.
• RN or BSN degree.
• 5+ years of clinical experience.
• Background in care management and/or chronic condition support.
• Strong telephonic assessment, clinical judgment, and communication skills.

Nice to have

• Behavioral health experience.
• Experience with GLP-1 medications and/or weight management programs.
• Additional state licenses in CA, NY, MI, MN, or OR.
