Salary: $100,000 – $110,000

What you will do

• Conduct initial assessments of clients to determine care needs and develop individualized care plans.
• Train and supervise non-skilled caregivers in providing personal care and daily living assistance.
• Monitor clients’ overall well-being and report significant health changes to healthcare professionals.
• Educate clients and their families on effective care strategies, safety measures, and daily living skills.
• Ensure full compliance with agency policies, state regulations, and best practices in non-skilled care.
• Maintain accurate documentation in EHR systems and act as a liaison between clients, families, and caregivers.

Requirements

• Bachelor's Degree (BSN) and current NYS License (or Associate's degree with relevant homecare clinical experience).
• 1-2 years of Nursing experience specifically within the homecare agency space.
• Valid driver’s license and reliable transportation for travel within the 5 boroughs.
• Experience in geriatric care or working with individuals with disabilities.
• Knowledge of state and federal home health regulations and the Uniform Assessment System (UAS).
• Bilingual proficiency in Spanish.

Nice to have

• Master's degree in nursing.
• 3+ years of homecare agency experience.
• Field experience in assessing patient care and case management.
