What you will do

• Manage a longitudinal panel of high-risk patients via telephonic and video encounters.
• Conduct initial assessments and develop Palliative Plans of Care for newly enrolling members.
• Provide consultative visits for advanced symptom management, complex goals of care, and prognostication.
• Evaluate hospital admissions for root-cause and support transitions of care to reduce readmission rates.
• Collaborate with interprofessional clinical teams and leadership to ensure seamless patient care.
• Provide on-call weekend and holiday coverage on a rotating basis.

Requirements

• Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state.
• Willingness to obtain additional state licenses as required by the business.
• Minimum 5 years in advanced practice, with at least 3 years specifically in Palliative care.
• ACHPN® credential or willingness to obtain it within the first six months of employment.
• Ability to flex between different U.S. time zones.
• Comfort with technology and the ability to succeed in a fast-paced, work-from-home environment.

Nice to have

• ELNEC® or EPEC® training.
• Prior experience in ICU, oncology, or geriatrics nursing.
• Prior home-based palliative care or program development experience.
• Experience in interprofessional team leadership.
