Chronic Care Manager (Nursing)

What you will do

• Manage a patient panel of 30-50 patients per month through proactive outreach and care coordination.
• Conduct chart reviews, patient calls, and documentation to generate billable care management encounters.
• Develop and implement detailed care plans for chronic disease prevention and intervention.
• Provide motivational coaching to patients to improve health outcomes and adherence to care plans.
• Attend mandatory clinical update meetings and supervisor check-ins to align on scope and performance.
• Ensure all activities are performed in a HIPAA-compliant environment.

Requirements

• Graduate of an accredited School of Nursing (LPN, LVN, RN, BSN).
• Current COMPACT license to practice as an RN/LVN/LPN held in the state of residence.
• Minimum of two years of clinical experience in a clinic, Med/Surg, Case Management, or home health setting.
• Proficiency with Electronic Medical Records (EMR) and Windows-based applications.
• High-speed internet (min 18mbps download/6mbps upload) and a reliable Windows or Mac computer.
• Excellent verbal, written, and listening skills.
