What you will do

• Introduce the Transitions of Care program to patients during inpatient stays.
• Schedule post-discharge follow-up appointments with primary care and specialty providers.
• Obtain clinical documentation from hospitals and provider offices.
• Submit Home Care and DME orders and track vendor follow-ups.
• Assist patients with technology preparation for telehealth visits.
• Collaborate with clinical teams to discuss patients at risk for readmission.

Requirements

• Must be fully bilingual in English and Spanish.
• Must be based in the USA.
• 2+ years of patient-facing or care coordination experience in healthcare, managed care, or home health.
• Experience working in a telemedicine environment.
• Ability to work assigned shifts within the Eastern Time zone.

Nice to have

• Managed Care or Health Plan experience.
• MA Certification or significant medical assistant background.
• Strong proficiency with electronic health records and Google Workspace.
