What you will do

• Guide members through virtual care onboarding and digital platform usage to ensure comfort and confidence.
• Manage post-visit coordination, including orders, referrals, and follow-ups with pharmacies, labs, and DME suppliers.
• Support real-time telehealth workflows and triage member needs across digital channels such as chat, phone, and portal.
• Utilize and provide feedback on AI agents to optimize clinical prompts and member interactions.
• Act as a member advocate, reinforcing health education and managing transitions of care, including ER discharges.

Requirements

• MA Certification (highly preferred; expired accepted) or equivalent MA training/experience (transcripts required).
• 2+ years of patient-facing experience, preferably in telehealth, primary care, or urgent care/hospital backgrounds.
• Ability to work M-F 11:30am-8pm ET and weekends based on business needs.
• Digital fluency with EHR, chat, telehealth, and scheduling platforms.
• Strong communication skills for building trust via video, phone, and written channels.

Nice to have

• Bilingual proficiency in English and Spanish.
• Experience in care coordination for older adults or patients with complex health needs.
