Outreach Representative

What you will do

• Conduct telephonic outreach to assess member needs and provide support.
• Educate members on their rights, responsibilities, and preventive health services.
• Assist members in navigating grievance and appeals processes.
• Recruit and enroll members into Disease Management Programs.
• Coordinate access to care by contacting providers and community agencies.
• Maintain accurate records of all member and provider interactions.

Requirements

• High School Diploma or GED required.
• Minimum 1 year of experience in Community Outreach or Customer Service.
• Strong active listening and problem-solving skills.
• Ability to interact sensitively with diverse populations.
• Effective time management and multitasking abilities.
• Proficiency in maintaining professional documentation.

Nice to have

• Associate’s Degree.
• Familiarity with healthcare or insurance industries.
• Experience working with Medicaid and/or Medicare populations.
