What you will do

• Perform comprehensive psychosocial assessments via phone and video for members and caregivers.
• Conduct clinical social work interventions, including brief therapy, safety assessments, and navigation of support issues.
• Assess and intervene on social determinants of health to maximize program effectiveness.
• Collaborate with an interdisciplinary team of clinicians, nurses, and care coordinators.
• Identify and escalate complex member cases to the Behavioral Health Medical Director.
• Document all clinical interactions within the electronic health record.

Requirements

• Master’s degree in Social Work.
• Active clinical license (LCSW, LMSW, or comparable) in any US state.
• Minimum of 5 years experience in behavioral health or care management fields.
• Must be located in a US state where hirify.global is offered.
• Ability to independently manage a remote schedule (M-F, 8:30am-5:00pm PST or MST).

Nice to have

• Bilingual proficiency in Spanish, Creole, or Vietnamese.
• Experience in Geriatric Behavioral Health or the health insurance industry (Medicare Advantage).
