Location: Hybrid and field-based in Cleveland, Ohio, with required fieldwork and travel to the hirify.global office at least twice a month.

Salary: $28.00–$33.00 per hour

What you will do

• Conduct initial in-person home assessments with patients and principal caregivers, ensuring that care plans and compliance requirements are clearly established.
• Monitor patient and caregiver support through monthly contacts and in-person visits no more than 60 calendar days apart.
• Oversee enrollment, credentialing, claims operations, prior authorization appeals, satisfaction, and program-integrity reporting.
• Provide individualized coaching, education, and feedback to caregivers to strengthen care-delivery competencies.
• Act as an escalation point, advocate for service quality, and coordinate updates to patient care plans and treatment plans.
• Maintain accurate, timely, complete, and concise case records, reports, and visit documentation.

Requirements

• Valid and active Ohio Licensed Social Worker license issued by the CSWMFT Board.
• Bachelor’s or master’s degree in social work from a CSWE-accredited program.
• At least two years of clinical experience, including one year working with older adults or adults with disabilities.
• Valid driver’s license and insured vehicle for travel within Ohio.
• Experience with case management, operational evaluation, outcome tracking, and working independently in hybrid and field-based environments.
• Strong verbal and written communication, adaptability, empathy, emotional intelligence, and accountability.
