What you will do

• Conduct intake and needs assessments for clients seeking assistance with housing, transportation, health, and financial services.
• Connect clients with appropriate resources based on individual needs and eligibility using platform-driven insights.
• Monitor and follow up on referrals to ensure successful client connection to services and the resolution of identified needs.
• Build and maintain relationships with community-based organizations (CBOs), providing feedback to ensure network standards.
• Perform network health monitoring to identify referral trends, partner responsiveness issues, and workflow barriers.
• Collaborate on cross-functional projects to improve system efficiency and enhance the care coordination experience.

Requirements

• Must be based in the United States.
• Experience as a Care Coordinator, Care Navigator, case manager, community health worker, or in social work.
• Proven ability to navigate social services and coordinate referrals for complex health and social needs.
• Experience monitoring referral activity and analyzing performance indicators to support network health.
• Ability to pass a comprehensive background check (criminal, employment, and education verification).

Nice to have

• Board-certified case manager (CCM®) certification or the ability to obtain one.
• Bilingual proficiency in Spanish.
