What you will do

• Screen members for Hypertension Clinic eligibility and provision blood pressure monitors.
• Conduct comprehensive hypertension assessments, including medical history, medication adherence, and lifestyle habits.
• Deliver intensive nutrition and lifestyle counseling focused on heart-healthy habits and smoking cessation.
• Coach members using motivational interviewing to set and achieve sustainable health goals.
• Coordinate multidisciplinary care with NPs, Clinical Pharmacists, Social Workers, and Primary Care Providers.
• Maintain accurate documentation of member interactions and synchronize care plans.

Requirements

• Must be based in the USA.
• Experience as a Medical Assistant or in a clinical/healthcare setting.
• At least 2 years of work experience in longitudinal chronic disease management (e.g., primary or specialty care).

Nice to have

• Spanish bilingual proficiency.
• Experience caring for Medicare patients.
