Choices Care Coordinator

What you will do

• Partner with members and families to identify support needs and direct services for health and independent living goals.
• Collaborate with clinical and social support colleagues to address physical and behavioral health needs.
• Conduct thorough face-to-face home visits to assess member status, including psycho-social and financial factors.
• Authorize appropriate home and community-based services based on established criteria.
• Ensure member needs are met through ongoing monitoring and service coordination.

Requirements

• Must reside in Maury, Williamson, or Giles Counties, Tennessee.
• Registered Nurse with an active Tennessee license (or NLC state) OR Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
• Minimum 2 years of clinical experience.
• Valid driver's license and ability to travel to member homes.
• Proficiency in Microsoft Office and email.
• Available for an 8:00am – 5:00pm EST schedule.
