What you will do

• Coordinate scheduling and referrals for patients to specialty providers throughout the healthcare system.
• Review radiology reports to support care pathway algorithms and formalized transition processes.
• Track and document program interventions and outcomes, ensuring timely communication to referring physicians.
• Serve as a direct resource for patients and families, providing education and emotional support.
• Collaborate with stakeholders on data collection for registry participation and quality reporting.
• Attend care conferences and tumor boards to ensure appropriate patient data is available.

Requirements

• High school diploma and at least one year of post-high school medical education (Associate degree preferred).
• At least one (1) year of clinical oncology health care experience.
• Comprehensive understanding of medical terminology.
• Proficiency in computer databases and software, specifically Excel and Word; prior Epic experience is preferred.
• Demonstrated organizational skills to manage multiple monitoring systems.
• Ability to function autonomously with a high level of judgment and decision-making.
