Community Health Manager

Salary: $79,000.00 - $85,000.00

What you will do

• Lead implementation of Rural Health Care Transformation strategies (Dementia Capable Workforce, Community Resource Coordination and Navigation, and Brain Health Education and Risk Reduction) for assigned states, including grant reporting and goal tracking.
• Build scalable partnerships with FQHCs, health systems, clinics, rural health providers, and community organizations to drive grant outcomes.
• Supervise the team implementing the Rural Health program in assigned states, including staff supporting professional and workforce development, resource coordination, and education initiatives.
• Coordinate infrastructure and resources with state stakeholders aligned to each state's priorities.

Requirements

• Bachelor’s degree required in public health, social work, health administration, nursing, gerontology, or a related field.
• 3-5 years of progressive experience in public health, community health, chronic disease management, aging services, health system outreach, or related fields.
• 5+ years of leadership experience managing teams, preferably in large-scale/state program settings.
• Demonstrated experience working with FQHCs, primary care practices, health systems, rural health providers, and community-based organizations.
• Proven success leading workforce development initiatives and clinical-to-community integration/care-coordination or navigation models.
• Delaware based / Remote employee
