Salary: $115,000 - $140,000 annually

What you will do

• Conduct primarily telehealth video visits with occasional in-home visits for local members.
• Perform comprehensive clinical assessments and develop evidence-based, whole-person care plans.
• Manage chronic conditions, identify care gaps, and coordinate with internal and external care teams.
• Utilize AI-enabled workflows to minimize administrative tasks and maximize patient interaction.
• Foster trust and empathy with members and their caregivers throughout their primary care journey.

Requirements

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
• Must be based in Alabama.
• 3+ years of outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics.
• Active BLS certification.
• Proficiency in video telehealth and electronic documentation systems.

Nice to have

• Experience with managed care models like STARS/HEDIS.
• Background in performing preventative care visits for Medicare populations.
• Experience supporting individuals with complex medical or social needs.
