What you will do

• Deliver patient-centered primary care within an interdisciplinary team, managing complex diagnostic dilemmas and chronic diseases.
• Supervise, teach, and provide quality oversight for a pod of Advanced Practice Providers (APPs).
• Lead weekly interdisciplinary rounds and readmission conferences for high-risk patients.
• Drive innovation by developing AI-technology and digital engagement tools to enhance care delivery.
• Create and manage quality measures and oversight for the clinical pod.

Requirements

• Board-certified in Internal Medicine or Family Medicine.
• Must be based in the USA.
• Demonstrated commitment to primary care and experience serving diverse patient populations.
• Strong belief in measuring and improving the quality of population health delivery.

Nice to have

• Experience supervising healthcare professionals or Advanced Practice Providers.
• Proven ability to drive innovation or run clinical programs in a dynamic environment.
• Experience with virtual primary care.
