Salary: $120,000–$155,000 base + bonus

What you will do

• Perform initial assessments and develop comprehensive Palliative Plans of Care for new members.
• Manage a longitudinal panel of 40-50 high-risk patients via telephonic and video encounters.
• Provide consultative visits focusing on symptom management, goals of care, and disease education.
• Coordinate medical orders for home health, DME, medications, and hospice transitions.
• Participate in weekly interprofessional team meetings and regular clinical huddles.
• Evaluate hospital admissions to identify trends and support transitions of care.

Requirements

• Must possess a current, active, and unrestricted NP or PA license in at least one U.S. state.
• Must be willing to obtain additional state licenses as required.
• Minimum 5 years in advanced practice, with at least 3 years in Palliative care.
• ACHPN credential or willingness to obtain within six months.
• Willingness to flex between time zones.
• Comfortable working within an interprofessional care team and using virtual communication tools.

Nice to have

• ELNEC or EPEC training.
• Prior ICU, oncology, or geriatrics nursing experience.
• Prior home-based palliative care or program development experience.
• Prior interprofessional team leadership experience.
