What you will do

• Implement individualized, multidisciplinary care plans focusing on recovery, personal strength, and harm reduction.
• Assist patients in identifying and seeking treatment for underlying mental and physical health conditions.
• Develop interpersonal, social, family, and independent living skills through evidence-based interventions.
• Provide education on substance use disorders, including triggers, cravings, and medication adherence.
• Deliver services independently in patient homes and other community settings.
• Document all interventions and make ongoing assessments of patient progress toward treatment goals.

Requirements

• Associate of Applied Science in Human Services or Behavioral Health from an approved institution is required.
• Must have reliable transportation and a valid Driver’s License.
• Experience providing one-on-one or group rehabilitation and recovery services.
• Sufficient knowledge of substance abuse populations and treatment planning.
• Strong interpersonal communication skills and ability to work independently under pressure.

Nice to have

• Bachelor’s Degree in Psychology, Social Work, or other related human service fields.
