What you will do

• Partner with members and families to identify and direct services for health, employment, and independent living goals.
• Conduct thorough face-to-face assessments to determine physical, behavioral, and social needs.
• Develop and maintain comprehensive Person-Centered Plans.
• Collaborate with clinical and social support colleagues to ensure long-term service provision.
• Authorize appropriate home and community-based services based on established criteria.
• Travel to member homes to monitor service delivery and member status.

Requirements

• Must reside in Davidson or Wilson County, TN or within 45 miles.
• Valid driver's license and ability to travel to member homes.
• RN, B.S. in social work, nursing, education, or human services, or meet QDDP/QIDP federal requirements.
• 2 years of experience in the healthcare industry or operations.
• Proficiency in Microsoft Office suite.
• Ability to pass a digital literacy assessment and required medical/immunization screenings.
