What you will do

• Conduct telephonic outreach and scheduled video visits for patients post-hospital discharge.
• Perform medication reconciliation, symptom screening, and order necessary labs or supplies.
• Build rapport with patients to explain the care model and connect them to essential services.
• Coordinate care across hospitals, pharmacies, and community providers.
• Collaborate effectively with the nursing and administrative teams.
• Meet individual performance targets while contributing to population health goals.

Requirements

• Certified Nurse Practitioner in Adult, Family, or Geriatrics.
• Active state licensure in South Carolina (SC).
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
• Direct experience engaging Medicare and Medicaid populations with complex conditions.
• Comfort in a digital-first environment and virtual care delivery.
• Excellent written and verbal communication skills.

Nice to have

• Active licenses in TX, NJ, PA, IL, NY, MA, MN, or LA.
• Bilingual in English and Spanish with cultural fluency.
• Familiarity with Google Suite and Slack.
