What you will do

• Perform thorough telemedicine-based comprehensive history and physical exams.
• Evaluate risk scores and identify gaps in patient care.
• Develop evidence-based, whole-person care plans addressing clinical and non-clinical barriers.
• Collaborate with Oncology Nurse Navigators and Community Health Workers to prioritize member goals.
• Review medical records and coordinate with external healthcare providers.

Requirements

• At least 3 years of NP experience.
• Minimum 2 years experience in adult oncology.
• At least 1 year of experience in telehealth.
• Compact RN license and unrestricted Advanced Practice license.
• Willingness to obtain additional state licenses.
• Proficiency with Google Suite, Slack, and Electronic Health Records (EHR).
