What you will do

• Conduct comprehensive health assessments for Medicare Advantage members.
• Deliver preventive care remotely via proprietary software and video platforms.
• Document medical history, physical health, and psycho-social status in EMR systems.
• Create and coordinate personalized care plans with primary care physicians.
• Perform point-of-care diagnostic testing and manage care transitions.
• Communicate effectively with interdisciplinary care teams to close gaps in care.

Requirements

• Must be based in New Jersey
• Active NJ Licensure as a Registered Nurse or medical doctor license
• Certified as an Advanced Nurse Practitioner in specialty areas
• Active DEA and CDS registration
• Minimum 1 year of clinical work experience in direct patient care
• Bachelor’s degree (BSN) from an accredited school of nursing
• Current Basic Life Support (BLS) provider card
