What you will do

• Serve as an executive-level Account Manager, partnering with practice leadership and hospital-based navigators.
• Synthesize claims data and clinical insights into high-impact operational workflows.
• Orchestrate quarterly Town Halls to align diverse stakeholders on shared performance goals.
• Architect and refine specialized protocols for SNF patient management to improve patient wellness metrics.
• Champion optimized transition of care strategies for complex patient populations.

Requirements

• Must be based in the United States.
• Bachelor's degree in a clinical or healthcare-related discipline.
• Active nursing credential (RN, LPN) or International Medical Graduate (IMG) recognition required.
• Minimum 5+ years of clinical experience in a complex healthcare environment.
• Minimum 6+ years of professional experience in a value-based care landscape (ACO, MCO, or large health system).
• Demonstrated ability to manage executive-level stakeholder relationships and lead complex strategic projects.
