Salary: $40–$47 per hour

What you will do

• Serve as the primary point of contact for families, providing proactive care and clinical triage.
• Perform comprehensive assessments of clinical, psychosocial, and discharge planning needs.
• Collaborate with an interdisciplinary team of MDs, APPs, social workers, and pharmacists to establish care plans.
• Coordinate care transitions following ED, inpatient, or urgent care discharges.
• Communicate care plans effectively to families and the broader healthcare team.
• Document all care management assessments and interventions accurately.

Requirements

• Licensed RN in at least one state with eligibility for multi-state licensure.
• Bachelor’s degree in nursing from an accredited university.
• 5+ years of pediatric experience required.
• Must reside in Tennessee, Missouri, Louisiana, or Illinois.
• Availability for 3x12 hour shifts (Mon-Wed or Wed-Fri) 7:00am-7:00pm CST.

Nice to have

• 1+ years of experience in care coordination or case management.
• Bilingual in Spanish.
• Familiarity with Medicaid regulations and Value Based Care (VBC).
• Experience in virtual care settings.
