Location: Work from anywhere in the US. Must be licensed in NY, WI, MN, IA or be willing to obtain these licenses (or have IMLC).

Compensation: $300,000 - $330,000 USD

What you will do

• Provide remote preventive, acute, and chronic care via video visits.
• Manage a dedicated panel of patients as their primary care physician.
• Perform population health initiatives including AWVs and risk capture.
• Support patients post-discharge to reduce hospital readmissions.
• Collaborate with Care Concierge and Clinical Operations teams to reduce administrative burden.

Requirements

• Board-Certification in Family Medicine.
• 3+ years of post-residency work experience.
• 2+ years of experience in an in-person primary care setting.
• Prior experience with EPIC EMR and virtual visits.
• Ability to work 3x12 hour shifts, overnight.
• Clean background and medical malpractice history.
