CHOICES Care Coordinator (Shelby County)

Location: Remote within Shelby County, Tennessee, with travel to members’ homes and community settings. Schedule: 8:00 a.m.–5:00 p.m. EST, with no on-call requirement.

What you will do

• Partner with members and families to identify supports and direct services related to health, employment, independent living, and community living.
• Collaborate with clinical and social support colleagues to address physical, behavioral health, and long-term service needs.
• Conduct objective face-to-face visits and assess physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Use established criteria to authorize appropriate home- and community-based services.
• Confirm that authorized services are delivered and member needs are being met.

Requirements

• Must live in Shelby County, Tennessee.
• At least 2 years of clinical experience; 3 years of clinical-setting experience preferred or stated in the role overview.
• Active Tennessee registered nurse license, or an active license in a Nurse Licensure Compact state of residence; alternatively, an unrestricted MSW license such as LCSW, LMSW, or LAPSW.
• Valid driver’s license; field coordinators must complete a TB skin test, and vehicle use requires compliance with driver safety, insurance, and motor vehicle record requirements.
• Strong customer service, communication, interpersonal, organizational, critical-thinking, time-management, and prioritization skills.
• Basic Microsoft Office, email, and digital literacy skills; a digital literacy assessment is required.
