What you will do

• Conduct thorough face-to-face home visits to assess physical, behavioral, functional, and psychosocial needs.
• Partner with members and families to identify supports for health, employment, and independent living.
• Collaborate with clinical and social support colleagues to manage long-term service needs.
• Utilize specific criteria to authorize and verify the delivery of home and community-based services.

Requirements

• Registered Nurse (RN) with an active Tennessee license (or Compact license) OR Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
• Must reside within Shelby County, TN.
• Minimum of 2-3 years of experience in a clinical setting.
• Valid Driver's License and willingness to operate a vehicle for business.
• Availability for an 8:00am – 5:00pm EST schedule.
• Ability to pass a digital literacy assessment and required medical tests/immunizations.
