Salary: $115,000 - $135,000 per year + bonus + equity

What you will do

• Counsel patients via telephonic outreach on medication adherence and address barriers like polypharmacy.
• Perform focused clinical chart reviews to inform patient interventions and evidence-based recommendations.
• Collaborate with Physicians and administrative staff to optimize Medication Management workflows.
• Translate patient insights into targeted recommendations to drive improvements in practice operations.
• Develop standardized resources, educational tools, and clinical protocols to improve quality measures (STARs, HEDIS).

Requirements

• Doctor of Pharmacy (PharmD).
• Valid pharmacist license in at least 2 of the following states: AL, KY, LA, MS, NC, KS, PA, UT, VA, WV, MI, DE.
• Must be based in the US and available to work between 8 AM - 7 PM EST.
• Minimum of 3 years experience in value-based care, ambulatory care, or community pharmacy settings.
• Proficiency in telephony platforms and various Electronic Health Record (EHR) systems.

Nice to have

• Familiarity with Medicare, HEDIS, and STAR ratings.
