What you will do

• Conduct patient outreach, inbound calls, medication therapy management, comprehensive and targeted medication reviews, medication reconciliation, and prior authorization reviews.
• Assess medication therapy for adherence issues, adverse reactions, ineffective or unnecessary medications, inappropriate dosing, and additional treatment needs.
• Provide asynchronous video consultations and follow-ups to educate patients and support participation in clinical programs.
• Document clinical recommendations and patient-care notes, send recommendations to provider offices, and follow up on implementation.
• Develop and update pharmacy-service policies, procedures, protocols, workflows, and operational improvements.
• Collaborate with nurses, health coaches, managers, product teams, experience teams, and client-facing staff; supervise direct reports when applicable.

Requirements

• Current pharmacist license and at least three years of pharmacy practice experience.
• Fluent English and Spanish, including reading, writing, speaking, and comprehension.
• Current residence in Puerto Rico and authorization to work in Puerto Rico and the United States without employer sponsorship.
• Ability to obtain pharmacist licensure in additional states as needed.
• Strong telephone and video communication skills, particularly for serving older adults, with excellent listening and interpersonal abilities.
• Ability to work independently in a remote, decentralized environment while protecting confidential patient information; proficiency with Microsoft Teams, Outlook, Word, Excel, and similar tools.
