What you will do
- Conduct intake assessments and develop individualized SMART self-sufficiency and wellness plans for clients.
- Perform critical screenings for mental health, trafficking, and legal needs.
- Assist clients in navigating NYC healthcare, shelter, and social service systems, including benefit applications.
- Provide referrals and maintain a strong network of community service providers to meet client needs.
- Manage detailed client documentation using the Efforts to Outcomes (ETO) database.
- Respond to client crises and accompany clients to relevant court or health service appointments.
Requirements
- Minimum 1-2 years of case management experience with asylum seekers or similar populations required.
- Must be based in the New York City area to perform office and field duties.
- Fluent English (spoken and written) required.
- Bachelorβs degree in social work, public health, or a related field (preferred).
- Proficiency in Microsoft Office applications (Word, Excel, Outlook).
Nice to have
- Proficiency in languages spoken by client groups, such as Spanish, French, Russian, Dari, Pashto, Arabic, or Wolof.
