What you will do

• Provide comprehensive primary care for patients managing complex and chronic illnesses.
• Manage diverse medical conditions, including diabetes, hypertension, obesity, and mental health.
• Deliver care using asynchronous text-based chat and synchronous video via a proprietary app.
• Collaborate with a clinical care team to optimize patient outcomes through shared decision making.
• Work flexible evening and weekend shifts (up to 8 hours per month).

Requirements

• Active state licensure in Texas (TX).
• 5+ years of advanced clinical experience in complex care, chronic care management, or primary care.
• Certification as a Nurse Practitioner.
• High proficiency in using digital healthcare platforms and tools like Google Suite and Slack.
• Ability to work up to 10 hours/week during the initial onboarding month.
