What you will do

• Provide high-quality preventative, acute, and chronic care in a fully remote virtual setting.
• Manage a dedicated panel of patients as their primary care physician.
• Refer patients to specialists or acute care services when clinically indicated.
• Support patients post-discharge to improve treatment plan understanding and reduce readmissions.
• Execute population health initiatives for risk-attributed patients.
• Practice continuous self-evaluation to ensure adherence to clinical guidelines.

Requirements

• Active Wisconsin, Minnesota, and Iowa medical licenses with IMLC compact licensure.
• Board-Certified in Family Medicine.
• 3+ years of post-residency work experience.
• Minimum 2+ years experience in an in-person primary care setting.
• Prior experience with EPIC EMR.
• Must be able to work Monday and Friday with specific shift flexibility.
