What you will do

• Conduct virtual visits with older adults and review medical histories.
• Identify health-related social needs (HRSNs) during the care planning process.
• Create and update care plans to guide Community Care Advocates in providing support services.
• Perform asynchronous chart reviews and manage clinical escalations.
• Collaborate with operational teams to design and refine clinical workflows.

Requirements

• Active, unrestricted MD or DO license in good standing.
• Board-certification in Internal Medicine or Family Medicine.
• Active Florida state medical license, Florida Medicaid credential, and Medicare enrollment.
• Minimum 2 years of post-residency clinical experience.
• Ability to work a minimum of 8 hours per week for synchronous consultations on weekdays.

Nice to have

• Prior experience with telemedicine.
• Fluency in Spanish.
• Licensure or credentialing in additional US states.
