What you will do

• Conduct face-to-face needs assessments in member homes and nursing facilities.
• Develop and implement individualized care plans for members with chronic and complex health needs.
• Coordinate care across physical, behavioral, and long-term service delivery systems.
• Facilitate transitions between hospital, home, and community-based care settings.
• Educate members and caregivers on available benefits, resources, and long-term care options.
• Document all interventions and case management services in the electronic health record.

Requirements

• Must be based in Delaware and possess reliable transportation for daily travel.
• Bachelor's degree in Social Work/Human Services with 3 years of experience OR Master's degree with 1 year of experience OR RN/LPN licensure with 2 years of experience.
• Ability to work flexible hours to meet member needs.
• Proficiency in PC-based word processing and database documentation.
• Understanding of cultural competency in addressing targeted populations.
• Ability to comply with all telecommuter policies and maintain a dedicated home workspace.

Nice to have

• Certified Case Manager (CCM) certification.
• Experience with HIV/AIDS, behavioral health, or developmental disabilities populations.
• Managed care, Medicare, and Medicaid experience.
• LBSW, LMSW, or LCSW licensure.
