What you will do

• Partner with members and families to identify support needs and personal health goals.
• Conduct thorough face-to-face home visits to assess physical, behavioral, and social needs.
• Collaborate with clinical and social support colleagues to ensure comprehensive care.
• Authorize appropriate home and community-based services based on established criteria.
• Manage daily tasks and prioritize member needs in a fast-paced environment.

Requirements

• Must live within Davidson County, TN or within 30 miles.
• Registered Nurse (active TN license or NLC) OR Masters in Social Work (LCSW, LMSW, or LAPSW).
• 2 years of experience in a clinical setting.
• Valid Driver's License and ability to travel to member homes.
• Available for an 8:00am – 5:00pm EST schedule.
• Proficiency in Microsoft Office and email.
