TL;DR

Crisis Care Manager (Medical): Coordinating care and providing emotional support for families of children with complex medical and behavioral needs with an accent on resource linkage and crisis intervention. Focus on navigating the healthcare system post-hospital discharge and addressing social determinants of health.

Location: Remote (Must be able to work CST shift hours: 8:00am - 04:30pm or 10:30am - 07:30pm)

Salary: Starts at $65,000 + annual bonus

A tech-enabled, pediatrician-led medical group reimagining care for children with special healthcare needs through virtual and in-home services.

What you will do

  • Provide emotional support and encouragement to patients and families experiencing behavioral health crises.
  • Handle case referrals, assess resource needs, review progress, and manage case closures.
  • Assist families in achieving recovery goals through psychoeducation and linkage to community resources.
  • Assess social determinants of health, including housing, income, food insecurity, and transportation.
  • Coordinate care with interdisciplinary teams, DCFS, school systems, and outside care providers.
  • Participate in scheduled consultations to monitor patient progress and facilitate treatment goals.

Requirements

  • Master’s degree in Social Work (MSW) required.
  • 2+ years of experience in case management or a relevant field.
  • Experience with acute behavioral health needs such as suicidality or severe persistent mental illness.
  • Background in child and adolescent case management, inpatient settings, or community mental health.
  • Strong collaboration skills and experience with team-based care.
  • Excellent working knowledge of community resources.

  • Bilingual proficiency in English and Spanish.
