What you will do

• Partner with members and families to identify support needs and direct services for health and independent living.
• Collaborate with clinical and social support teams to address physical and behavioral health requirements.
• Conduct thorough face-to-face visits to assess member status, including functional, psycho-social, and financial needs.
• Authorize appropriate home and community-based services based on established criteria.
• Maintain accurate documentation and ensure service delivery meets member needs.

Requirements

• Must live within Shelby County, Tennessee
• Registered Nurse with an active Tennessee license or NLC compact license, or Master in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
• Minimum 3 years of experience in a clinical setting.
• Valid driver's license and ability to travel to member homes for visits.
• Available for an 8:00am – 5:00pm EST schedule.
• Digital literacy and proficiency with Microsoft Office and email.
