What you will do

• Conduct remote nursing assessments via phone and video visits for members with heart failure.
• Provide tailored clinical education on disease processes, medications, and symptom monitoring.
• Develop and evaluate personalized care plans in partnership with members and caregivers.
• Collaborate with an interdisciplinary team of nurse practitioners, pharmacists, and social workers.
• Coordinate care with primary care providers to ensure evidence-based treatment.
• Contribute to operational improvements and the development of a virtual Heart Center of Excellence.

Requirements

• Active multi-state Compact RN license required.
• Must be based in the Mountain (MT) or Pacific (PT) time zone.
• Minimum 4 years of RN experience, including at least 2 years in a non-ICU cardiac setting.
• Strong verbal communication skills and ability to multitask in a fast-paced environment.
• High comfort level with technology, including EMRs and AI-based messaging tools.
• Secure, private workspace with reliable internet connection.

Nice to have

• Experience in telehealth or remote clinical roles.
• Bilingual proficiency in Spanish.
• Experience working within the Medicare Advantage landscape.
