What you will do

• Screen members for Diabetes Clinic eligibility and conduct detailed clinical assessments.
• Provide ongoing education on disease processes, medications, glucose monitoring, and lifestyle counseling.
• Collaborate with nurse practitioners, social workers, and pharmacists to develop and optimize care plans.
• Utilize motivational interviewing to influence positive, sustained behavioral changes in members.
• Maintain accurate documentation of all member and provider interactions.
• Connect members with internal resources to troubleshoot clinical issues and close gaps in care.

Requirements

• Active unrestricted Compact Registered Nurse license or Registered Dietitian license required.
• Active Certified Diabetes Care and Education Specialist (CDCES) certification required.
• Minimum 2 years of clinical diabetes management experience.
• Must be based in one of the contiguous 50 United States.
• Ability to obtain additional state licenses as required by the company.

Nice to have

• Experience managing diabetes in a remote setting.
• Proficiency with diabetes technology including CGMs and insulin pumps.
