What you will do

• Conduct focused virtual assessments (80–90% of time) for general cardiology and heart failure needs, including ordering diagnostics, interpreting results, and building treatment plans.
• Diagnose and manage heart failure and cardiology conditions, including medication management, volume monitoring, and lifestyle modifications.
• Initiate, titrate, and manage guideline-directed medical therapies (GDMT) for heart failure patients.
• Manage acute volume overload and weight fluctuations to reduce risks of exacerbations and hospitalizations.
• Order and coordinate in-home diagnostics (echocardiograms, EKGs, Holters) and partner with community cardiology practices for additional testing.
• Maintain accurate medical records, support quality improvement, and provide clinical escalation support during business hours.

Requirements

• Licensure at time of hire: active and clear RN and APRN or Physician Assistant license in Texas, Ohio, Arizona, Illinois, Georgia and/or Alabama, plus APRN certification; must be maintained while employed.
• Bilingual clinical assessments in Spanish and English are required.
• 5+ years of cardiology clinical practice experience (ideally 2+ years in outpatient cardiology).
• Experience delivering visits via telehealth video platforms and strong desire to continue practicing virtually.
• Ability to work 8–5 in Central Time or later time zone (MT, PCT).
• Willingness to obtain and maintain multistate licensure within the first 90 days (support and reimbursement provided).

Salary: $120K–$155K base range plus performance-based bonus ($10K–$20K) for total comp range of $130K–$170K
