What you will do

• Deliver remote clinical monitoring and member education via software and video conferencing.
• Analyze data to promote member self-care and evaluate progress toward health goals.
• Monitor health condition reversal trends and support medication management.
• Collaborate with an interdisciplinary team of nurses, health coaches, and providers.
• Triage member alerts and inquiries using established nursing protocols.
• Maintain accurate medical records in compliance with federal law and company policies.

Requirements

• Active and unencumbered RN license in the state where patient care occurs.
• BSN or MSN from an accredited school of nursing.
• Minimum 4 years of nursing experience across various healthcare systems (hospitals, clinics, or payor systems).
• Must be based in the USA.
• Proficiency with electronic documentation systems and business tools like Google and Slack.
• Ability to manage large caseloads in a fast-paced, high-growth environment.

Nice to have

• Experience in case/care management, population health, or value-based care.
• Skill in motivational interviewing and driving behavior change.
• Fluency in Spanish.
