What you will do

• Provide clinical triage, support, and education to members via phone and email.
• Conduct comprehensive clinical assessments and oversee member health and longitudinal care.
• Coordinate care across the oncology continuum, from screening to survivorship or end-of-life care.
• Collaborate with Nurse leaders and Medical Directors to ensure alignment with clinical protocols.
• Partner with payers and oncology/primary care providers to optimize care delivery.
• Work with non-clinical team members to address social determinants of health, such as food and transportation.

Requirements

• Active, unrestricted compact Registered Nurse (RN) license.
• Degree in Nursing (ADN or BSN required; BSN preferred).
• At least 5 years of nursing experience, including 3 years in solid tumor oncology nursing.
• OCN, AOCN, AOCNS, or CCM certification (required or obtained within 2 years of hire).
• Must be located within the lower 48 United States due to PHI access restrictions.

Nice to have

• Compact and single-state licensure in New York and/or Illinois.
• Fluency in Spanish, both written and verbal.
