RN, Senior LTSS Service Care Manager

Location: Hybrid role with remote work from home and required member home visits in Hidalgo County, Texas, near Donna, Alamo, Pharr, San Juan, McAllen, or Weslaco. Schedule: Monday–Friday, 8:00 AM–5:00 PM CST. Travel is required for member visits.

Pay range: $36.21–$65.09 per hour.

What you will do

• Assess complex and high-acuity members and recommend care plans supporting quality and cost-effective outcomes.
• Develop, monitor, and update personalized long-term care and service plans.
• Coordinate medical services, providers, specialists, facilities, agencies, and community resources.
• Monitor member conditions, complications, progress, care gaps, waiver eligibility, and potential needs for a higher level of care.
• Educate members and families or caregivers about disease processes, benefits, care options, referrals, and available community services.
• Conduct required home or other site visits, document care management activities, and support quality improvement and service delivery.

Requirements

• Graduate of an accredited school of nursing.
• 4–6 years of related experience, preferably in RN care coordination or case management with adults aged 21 or older in Home Health, Hospice, Long-Term Care, or Managed Care.
• Current RN state licensure or compact state licensure; current state nurse licensure is also accepted for Nurse Practitioners.
• Bachelor of Nursing degree preferred.
• Must support member visits in Hidalgo County, Texas, and travel for assessments.
• For Superior assignments, Resource Utilization Group certification must be obtained within 90 days of hire.
