What you will do

• Provide longitudinal nursing support for members with behavioral health concerns ranging from coping support to severe mental illness.
• Conduct telephonic clinical assessments, including pain assessments and medication reconciliation.
• Coordinate with Social Workers and Community Health Workers to address social determinants of health (food, transportation, home support).
• Develop targeted support plans and participate in interdisciplinary case conferences to align care plans.
• Manage member queues and documentation using Slack, EHR, and other internal tools to ensure timely follow-up.

Requirements

• Bachelor of Science in Nursing (BSN) required.
• Multi-state, unrestricted Registered Nurse (RN) compact license.
• Minimum 5 years of nursing experience.
• At least 2 years of experience in advanced clinical judgment for complex behavioral health cases.
• Proficiency with modern technology (Google Suite, Slack, electronic health records).

Nice to have

• Experience in oncology nursing.
• Oncology Certified Nurse (OCN) certification.
