What you will do

• Supervise and monitor the Promotora de Salud (PdS) team via regular 1:1s and performance reviews.
• Develop and implement tools to document performance and track program goals.
• Ensure timely and complete documentation of team activities and implement quality assurance measures.
• Coordinate community events and serve as a stakeholder in program model changes.
• Facilitate case conferencing and provide on-call consultation to support direct reports.
• Manage member feedback and escalate issues to improve products and services.

Requirements

• Bachelor’s Degree or equivalent experience in social services.
• Must reside in San Bernardino, CA.
• 3+ years in a healthcare setting, including experience with Medicaid/Medicare.
• 2+ years of experience as a CHW/PdS and 2+ years as a supervisor to frontline staff.
• Fluent in English and Spanish (verbal and written).
• Authorization to work in the United States.
